The force has self-referred to the ICO and is <a href=\"https:\/\/www.telegraph.co.uk\/news\/2026\/08\/15\/met-mistakenly-exposed-143-fayed-victims\/\">facing a potentially large fine<\/a>. It&#8217;s the same mistake that shows up over and over: <a href=\"https:\/\/www.commercialappeal.com\/press-release\/story\/143128\/kiteworks-doubles-down-on-its-data-control-plane-commitment-with-launch-of-agent-and-human-error-prevention\/\">misdirected email is still the single largest breach category reported to the UK ICO<\/a>, and Verizon finds the human element in 62% of confirmed breaches.<\/p>\n<p>That&#8217;s why a useful outbound-email safeguard has to do more than ask, \u201cAre you sure?\u201d &#8211; it should help the sender verify the actual address before the message leaves Microsoft 365.<\/p>\n<h2>Why display names create a blind spot<\/h2>\n<p>Outlook\u2019s autocomplete is built for speed.
